CLDN18.2: China approved zolbetuximab for first line care - what it means for Indonesian patients

China's regulator, NMPA, approved zolbetuximab in June 2025 as first-line treatment with chemotherapy for advanced CLDN18.2-positive, HER2-negative gastric or gastroesophageal-junction adenocarcinoma. The decision changes what may be worth discussing, but it does not automatically mean the medicine is available in Indonesia or at every hospital in China.

This is not news and not a medical review

This explainer is anchored to one verified regulator decision, the NMPA announcement accessed on 2026-09-15. Approval in one country does not mean the medicine is available, affordable, or approved elsewhere.

Medical review status

This is a standing explainer, not breaking news and not a medical review. It has not been reviewed by a doctor and does not replace the explanation of the medical team caring for the patient. Not a diagnosis or final medical recommendation. Requires physician review.

What was approved: regulator fact

On 11 June 2025, China's NMPA approved zolbetuximab for injection, trade name VYLOY, for marketing in China.

The official indication states use with fluorouracil- and platinum-based chemotherapy as first-line treatment for unresectable or metastatic CLDN18.2-positive, HER2-negative gastric or GEJ adenocarcinoma.

Zolbetuximab is a monoclonal antibody targeting CLDN18.2. TanyaKanker cites the regulator wording without changing its meaning.

What is still uncertain: availability, cost, and Indonesia status

Approval, availability, and payment coverage are separate. NMPA approval applies to China; Indonesian approval and access must be confirmed through the doctor or care facility.

  • Availability: whether a destination hospital actually provides it for international patients must be confirmed directly.
  • Criteria: CLDN18.2-positive, HER2-negative, advanced, first-line status is assessed by the doctor using clear marker results.
  • Cost: TanyaKanker does not publish a price. Ask the destination facility for a written component estimate.

The CLDN18.2 journey in stomach cancer

From tissue testing to the questions worth asking after the result is available.

This diagram explains the sequence of assessment and discussion. It is not treatment advice or an eligibility decision.

Read the CLDN18.2 marker explainer
  1. Stomach cancer cells are examined

Assessment is performed on tumour tissue, not blood.

  2. CLDN18.2 testing

The result is usually reported with a staining percentage and intensity.

  3. Positive or negative result

Assessment thresholds may differ between facilities.

  4. Options are discussed

Including options approved in a country and those still under investigation.

  5. Clinical trial or second opinion

The next step depends on the records and the patient's condition.

A marker name in a report is the start of a discussion, not a decision.

What this means for discussion with the doctor

For families with a CLDN18.2-positive result, this approval adds a real option to discuss, not a promise of access for the case.

Useful questions include whether CLDN18.2 has been tested with a clear score and threshold, whether HER2 has been assessed, and which options fit the combined results.

Limits of this explainer

The verified fact is the Chinese regulator decision and its stated indication. This article does not assess individual suitability, promise an outcome, or recommend treatment.
