EGFR after osimertinib: fourth-generation trials are starting to report early results
No fourth-generation EGFR targeted therapy has been approved anywhere. Candidates such as JIN-A02 are being tested in phase 1/2 for patients who progress after osimertinib, including cases with C797S resistance. The published early results are small and are not proof of efficacy.

This is not news and not a medical review
Medical review status
Short answer: none is approved yet
For EGFR-mutated NSCLC that progresses after osimertinib, no fourth-generation EGFR targeted therapy is approved as the next step.
Current options include early-phase candidates in trials and established routes such as chemotherapy or combinations chosen with the doctor. Early data is not the same as an available medicine.
Facts from the trial registry
One candidate is JIN-A02, an oral drug described in the registry as a fourth-generation EGFR TKI. TanyaKanker rechecked ClinicalTrials.gov NCT05394831 on 2026-09-15.
- Design: phase 1/2, open-label, multicenter; this phase mainly assesses safety, tolerability, and dose.
- Population: advanced NSCLC with EGFR mutation after standard anticancer therapy, including approved EGFR TKIs such as osimertinib.
- Registry status: recruiting. Recruiting does not mean international patients are accepted.

The EGFR journey after targeted therapy stops working
The sequence usually discussed when EGFR-targeted therapy no longer controls the disease. It covers tests and questions, not drug recommendations.
This diagram explains the sequence of assessment and discussion. It is not treatment advice or an eligibility decision.
An EGFR alteration is found
Recorded in the molecular report, not only in its conclusion.
Targeted therapy is given
Including the start date, dose, and reason for changing treatment when relevant.
The disease progresses
Assessed from imaging and the clinical condition by the treating doctor.
Possible resistance mechanisms
For example C797S, MET, or another alteration that may be found on repeat testing.
Repeat testing or NGS
Determines which changes are actually present in this case.
Discuss the next step
Reviewed with the doctor according to the patient's condition and available options.
When targeted therapy stops working, it does not automatically mean there are no further options to discuss.
Published early results and their limits
The Clinical Cancer Research paper reports preclinical data and an early clinical slice. Laboratory models showed activity against C797S and T790M, including brain tumor models.
For the first human dose-escalation cohort, 23 patients had been treated by the data cutoff, with three partial responses and seven cases of stable disease.
Numbers this small describe early safety and dose finding, not a comparison of benefit against other treatment routes.
What can be discussed with the doctor now
The entry point after progression is usually repeat molecular testing, through tissue biopsy or liquid biopsy when appropriate, to look for the resistance mechanism.
The mechanism, not the drug name alone, guides the next discussion. Trial records that look relevant still need direct confirmation from the study team.
