Next-generation sequencing (NGS)
A test that reads many genes from one tumour sample to find actionable changes.

Not a diagnosis and not final medical advice. Must be reviewed by a doctor.
Purpose of the test
- Find mutations, fusions and gene copy changes in one test.
- Identifying markers that could open up clinical trial options.
Informs decisions
- Categories of targeted therapies that are worth discussing.
- Compliance criteria for molecular marker-based clinical trials.
How the test is done
DNA and RNA from tumor tissue are read with a sequencing platform, then interpreted together with clinical data.
- Sample type
- Tumor tissue block or blood sample (if using a liquid panel)
- Estimated result time
- Generally one to three weeks, may take longer if orthogonal confirmation is needed.
Limitations
- Different panels examine different lists of genes and types of changes.
- Not all of the changes found have available therapy.
- Some findings require confirmation by other methods.
- Results need to be read together with pathological findings and clinical conditions.

EGFR after osimertinib
My treatment stopped working. What does the next step depend on?
- EGFR markerStarting point: what is tested and what the marker does not decide by itself.
- EGFR exon 19 deletionA variant that often makes EGFR inhibition relevant to discuss.
- OsimertinibThe drug that most often forms the context for this question.
- Progression after targeted therapyThe decision situation you face, not a disease name.
- Osimertinib resistance guideThe order of questions and steps when treatment stops working.
- NGS (next-generation sequencing)A test that can assess many genes from one sample, including resistance mechanisms.
- EGFR C797SOne resistance mechanism looked for on repeat testing.
- MET amplificationAnother resistance mechanism that can change the direction of discussion.
- EGFR T790MA mechanism where treatment sequence matters.
A structured summary so repeat testing and treatment history are readable.
